Ensuring your healthcare wishes are honored is a vital step in planning for the future. The Arizona Healthcare Directives Registry (AzHDR) offers a secure way to make your advance directives accessible to healthcare providers statewide. Below are the steps to successfully register your directives in the AzHDR.
Step 1: Complete the AzHDR Registration Agreement
Start by filling out the AzHDR Registration Agreement, which must accompany your advance directives. This document must be signed and notarized to be valid. To ensure your registration process goes smoothly, make sure all information is legible and clearly printed. Accurate and complete details are essential to avoid delays.
Step 2: Submit Your Documents to Contexture
Contexture is the provider of the registry service. Once your Registration Agreement and advance directives are ready, you’ll need to send them to Contexture. Be sure to only submit copies of your documents rather than the originals, as these cannot be returned after being uploaded into the registry.
You can send your forms either by mail or email:

Step 3: Review and Confirmation Process
After receiving your submission, Contexture will review your directives to ensure they comply with Arizona law. You will be notified by mail once your documents have been processed. If any corrections are required, the letter will include detailed instructions on what needs to be addressed.
Once your directives are approved, Contexture will send you login information for accessing your records in the AzHDR. You’ll also receive details about how to complete your wallet card, which provides quick access to your registry information in case of an emergency.
Step 4: Your Directives Are Published
Once you’ve received confirmation that no corrections are needed, your advance directives will be officially published in the AzHDR. At this point, no further action is required on your part. Your documents will be accessible to authorized healthcare providers participating in the registry, ensuring that your wishes are known when they matter most.
